First, a family contacts Jenn and their concerns and needs are discussed, including whether Jenn can provide the type of evaluation needed to best answer their questions. In the case of separated or divorced parents, both parents need to agree to the evaluation.
Next the family completes a detailed questionnaire that delineates background history and developmental milestones. These forms are available on this website. Completed forms can be brought to the first appointment, or they can be scanned, emailed, or faxed ahead of time. Previous assessments, IEPs, and relevant reports should also be brought to the first appointment.
At the first appointment, Jenn meets with parents for an intake session. This session provides an opportunity for parents to meet with Jenn alone to discuss their concerns, ask questions, and get to know Jenn. Intakes generally last about an hour, and typically involve a complete review of the child’s developmental history, various questionnaires provided in advance and discussion about parental observations of the child.
The next appointment(s) are for the administration of the formal testing. Depending on the type of assessment, and various client factors (fatigue, illness, etc.) the testing will be completed in one or two sessions of several hours.
After the testing is complete, there is a feedback session for parents. This session is designed to present findings to parents/guardians, respond to the referral question(s), and fully explain recommendations. Adolescents and children may have a separate feedback session scheduled; this appointment and associated costs will be discussed ahead of time. A full report with findings and recommendations is provided to you approximately 2-3 weeks following the feedback session, depending on when the feedback appointment is scheduled. Except for cognitive testing for school admission, reports include background, test results and interpretation, any diagnoses, and recommendations.
